Upon referral, a nurse or therapist visits with the patient to assess their needs to develop an individualized care plan. Under the direction of a physician, a care team will deliver the necessary services using the latest techniques to maintain or restore patient independence. Park Ridge Home Health services include:

Park Ridge Home Health services are available 24 hours per day, seven days per week. Administrative offices are open Monday through Thursday from 8 a.m. to 5 p.m. and on Friday from 8 a.m. until 4:45 p.m.

Convalescence from surgery or illness often may occur comfortably at home with appropriate medical equipment. Park Ridge Health Home Medical Equipment is committed to providing equipment and services that meet or exceeds the standards of our industry. For your convenience, medical equipment is scheduled, delivered to your home, and assembled by our fully-trained, certified staff. We are available to provide the following Home Medical Equipment:

Project C.A.R.E.

Park Ridge Health is the host organization Project C.A.R.E. (Caregiver Alternatives to Running on Empty), which offers critical respite care, family consultation, and community resources to family caregivers of persons with Alzheimer's disease and other types of dementia. Park Ridge Health is dedicated to meeting the critical needs of families caring for people with dementia, while ensuring high standards of community-based service and a supportive environment for Project C.A.R.E.

Project C.A.R.E. is now being offered in 22 counties across the North Carolina. The program especially strives to reach low-income rural and minority families.
